Multimodality Imaging in Transcatheter Mitral Interventions

چکیده

Both echocardiography (specifically transesophageal echocardiography [TEE]) and multidetector row computed tomography (MDCT) have played an integral role in preprocedural planning for transcatheter aortic valve replacement, heavily influencing the moniker of “multimodality imaging.” The strengths of each modalities are complementary (Table), although there is overlap between the preprocedural analyses which can be performed. Having 2 modalities capable of accurately measuring the same anatomic structure has clinical utility. For instance, mandated MDCT annular measurements could not be performed accurately by the core laboratory in 6% to 8% of patients in the Placement of Aortic Transcatheter Valves II SAPIEN 3 trial and thus required 3-dimensional (3D)-TEE for transcatheter valve sizing.

Role of echocardiography in percutaneous mitral valve interventions.

Intraprocedural imaging continues to evolve in parallel with advances in percutaneous mitral valve interventions. This didactic review uses several illustrations and rich intraprocedural videos to further describe and demonstrate the role of the most up-to-date echocardiographic and advanced imaging technologies in the patient selection and intraprocedural guidance of percutaneous mitral valve ...

High-risk Coronary Artery Bypass Grafting and Mitral Valve Replacement in a HIV Positive Patient

Certain subsets of high-risk mitral valve patients are not suitable candidates for transcatheter therapies. The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...

The Year in Cardiology 2012: valvular heart disease.

Growth of the ageing population, advances in imaging techniques, newly available percutaneous interventions, and emphasis on the central role of the Heart Team in risk assessment and treatment selection combine to place valvular heart disease (VHD) at the centre of developments in cardiovascular medicine and surgery. However, VHD is poorly researched in comparison with other areas of cardiac di...

Percutaneous transcatheter closure of prosthetic mitral paravalvular leaks: are we there yet?

A potential complication of mitral valve replacement surgery is the development of a paravalvular leak (PVL). Percutaneous transcatheter closures of PVLs using a wide array of devices have been reported in the literature, although the procedural success rate of this approach remains variable. One major challenge of transcatheter mitral PVL closure lies in the ability to adequately visualize the...

Multimodality imaging in the cardiac catheterization laboratory: a new era in sight

Multimodality imaging in the cardiac catheterization laboratory continues to develop with rapid advancements in the use of intracardiac echocardiography (ICE), 3D echocardiography (3DE) and multidetector computed tomography (MDCT), with fluoroscopic overlay to produce 3D images of intracardiac structures. The improvement in these imaging tools have coincided with innovation and procedural advan...
